Hi Josef! After a bit of researching, I've decided to remove the "personal" option for licensing, and instead use the Creative Commons family of licenses. This is because I realized that Creative Commons actually has quite a range of licenses.

Also, I've found the Creative Commons license you were looking for: CC-NC-ND. If you look at the legal code, you will see that it specifically prohibits performance of the work (section 1(a) and 3; 4(a) basically confirms 3). I know reading a real license may be a daunting thing to do, but I believe it is in your best interest to do so if you would like to avoid legal trouble in the future. In any case, I believe that a real license, such as the CC-NC-ND, will be much more comprehensive at covering all the aspects of the licensing compared to a "personal" makeshift license :)

Also, because the "personal" license category is going to be removed, you will have to re-license your work into a Creative Commons license if you want it to remain on IMSLP. Sorry for all the trouble regarding this situation, but I believe it is in the best interest of all of us to sort these legal issues out in the best way possible, instead of having to fix things after problems occur.

Publisher designation, etc.

Hi Josef, I'm Carolus, the copyright person (or at least one of the copyright people) here at IMSLP. As you know, much has happened since we went down last fall. One of the legal facets we've discovered is that the act of "publication" as far as the legal definition goes in Canada (and the US) includes having files available for download at a site like this one. Shall we list yourself as the publisher, with a direct link back to your own website (as I set things up on the Schaumkronen page)? Or, is there another publisher who we should be linking to? We are delighted to have your work available here and wish to assist you as much as possible in the promotion of performances, recordings, etc.

Dear Carolus, certainly I know you and your work here from the forum. Thank you for your kind offer. Yes, please list me as my own publisher and link to my website as done in the Schaumkronen. In fact, I'm glad for an example like this which I can abide by with future uploads. Since you might be busy enough, maybe I can change my other work pages myself? Yes, those are the fields that I would change. I very much appreciate you doing it also, in light of the large amount of tagging we still need to finish. Also, if you happen to have links to mp3s or similar audio files, it's easy to put those in as well in the Misc Notes: field in front of the CopyrightPermGranted template.
