This should be your name - not "Autograph manuscript" since what you added was not a scan of the manuscript but a new arrangement made by you. You can certainly insert "Based on the autograph manuscript" in the "Misc Notes" field, which would no doubt be appreciated. Also, as with a composer, the arranger is automatically assumed to be the primary editor so the Editor field should be left empty unless someone else edited your arrangement. If you use the new upload tools described on the Composer Portal, much of this kind of thing is done automatically so you don't even have to think about it. Of course, a scan of the manuscript would be nice to have also, so maybe your RISM number will encourage someone to add it.
