Misc. Notes Autograph according to entry in Worldcat. (Actually, the sheer number of crossouts and other corrections suggest this is either an autograph, or a copy by a very poor copyist, or...) (Probably not the revised finale though- it's all in one bundle.)

Misc. Notes Typeset using LilyPond 2.16.1. Other parts and perhaps a typeset piano arrangement also to follow in time. (Drafts, typesets still in progress.) Navigation etc.

"Opus 4" is just the general name of the website on which this is found; the symphonies in C and in E minor also downloadable from the same website also have "Opus 4" on the same page. There is no opus 4 on the scanned score. (Actually, this seems to have been finished sometime between the first piano quartet (op.3, 1832) and the first string quartet (op.13), in 1833.)

Re 1832 October see Composer's Thematic Catalog 1832-1837, page 5 (entered in his catalog on 22 October 1832/July 1833).
