Could this be Op.58 published 1888 by Hansen? The key is right. Yes. The holograph has 3 movements, the published score 4, a third movement "Passions-Intermezzo." in E♭ minor having been inserted.
Op.58 has been digitized by DKB; comparing the two works, yes, they are identical to within... so... uploading the printed score.
